Akton recruitment is working closely with the local authorities and we are are looking for an experienced Reablement Officer in the Newham area.
Overall Purpose of Job
To plan and assist the Service Manager, Senior Physiotherapist or Senior Occupational Therapist and other managers, in managing the delivery of all required rehabilitative services within their assigned area, in a competent and professional manner, ensuring compliance with Newham social care Reablement objectives, which are:
* To provide Reablement service to all appropriate people in their home
* To provide an initial assessment of need and Reablement potential
* To establish goals and required outcomes in agreement with the individual concerned
* To monitor reablement goals being delivered by third sector providers ensuring that the citizens of Newham have access to high quality services which safeguard and promotes their health & wellbeing.
* To provide regular reviews of progress towards outcomes
* To provide appropriate support towards achieving outcomes
* To liaise and work collaboratively with other health, social and third sector services to meet people's needs
* To recommend and/or arrange on-going support/advice/sign-posting for individuals on discharge from the service, as appropriate
* To provide equipment and assistive technology to meet assessed need
To undertake all responsibilities listed below:
1. To be aware of Care Quality Commission's Key Lines of Enquiries KLOE and embed this in-service delivery.
2. To be conversant with the Care Act and legislation governing the service.
3. To work in accordance with LBN and the Adults Reablement Provider Policies & Procedures, and with any further instructions given by the Management or Senior Management.
4. To be a "trusted assessor", identify, and commission non-complex equipment and adaptations required to support the individual to maximise their independence.
5. To attend and contribute to one to one supervision and appraisal sessions with the Line Manager, and ensure that they are informed, of any circumstances requiring management oversight including issues around risk, mental capacity and safeguarding practices
