Salary: Up to £30,000 per annum
Working Hours: Monday - Friday, 8:00 AM to 4:00 PM
We are currently seeking a dedicated and experienced individual to join a fast growing domiciliary care organisation as a Care Coordinator in Carmarthen. As a Care Coordinator, you will play a crucial role in ensuring the effective coordination of care services to enhance the quality of life for the clients.
Key Responsibilities:
Coordinate and schedule care services for clients in a timely and efficient manner.
Liaise with care staff and clients to ensure service delivery meets the highest standards.
Conduct assessments and reviews to determine the appropriate level of care required.
Maintain accurate records of client information and care plans.
Collaborate with healthcare professionals to provide comprehensive care solutions.
Respond to queries and concerns from clients and their families.Qualifications and Experience:
Previous experience in care coordinating is essential.
Full UK driver's license is mandatory.
Strong organisational and communication skills.
Ability to work independently and collaboratively in a team.
Knowledge of care regulations and best practices.Working Hours:
This is a full-time position with working hours from Monday to Friday, 8:00 AM to 4:00 PM.
Salary:
The salary for this position is up to £30,000 per annum, depending on experience and qualifications.
